Missing The Fiz-Sometimes
Just like a car. Both brakes every time (unless I'm creeping along in traffic and need to stop from 5mph or less).

Paraphrasing from the MSF guide,
Apply both brakes all the time. (you must continually practice and develop the the skill to prevent the f/r tire from locking up under emergency braking conditions)
If the front locks - release
If the rear locks - tougher call, if you release and the f/r are not in alignment you will high-side, if you keep it locked you might low-side. If you release in alignment you might get out of it alright.
Keep safe.

On a dry road use 75% of the front brake and 25% of the rear wheel.
When the surface of the road is wet use 50% to each wheel.
Under 10 mph use the back brake.
Practice braking in a empty parking lot so you get the feel of the brakes.
Always remember that the maximum braking effort is just before the wheel locks, if the brakes lock release and then reapply more gently.
I usually practice an emergancy stop on the exit ramp from a motorway if it safe to do so, that way I get the feel of the brakes in a controlled situation and I will understand how they work in an emergancy.
Dont forget you also can use your gears to maximise your stopping.
