The famous OSO2K on this very forum - do a search - there are million posts and installation instructions. The latest/easiest way is to buy a new joint (which I did), drill it, and then it directly replaces the existing one.

I can post pics later, but they're a completely new black version.

this gives you a pretty good idea.
When I did my install I added a small threded extension to keep the shift lever and the gear shaft levers both at right angles to the rod.

This mod brings you feet up and back about 1.5 inches. keeps your knees in tight on the tank!
you feel more "in" the bike.
